Informed consent on the collection of personal information
Clinique Alta physio cares about the confidentiality of your personal information. Your personal data is used only for the intended purposes and with the aim of providing you with efficient and coordinated care.
The following general identification information is mandatory for our files under our professional order:
First and last name, gender, date of birth and patient contact details.
The following information is optional but allows us to communicate effectively with the attending physician or his administrative staff. It is mandatory in a file involving CNESST. This information also allows us access to the electronic medical record for the purpose of verifying imaging reports or a medication list with your consent.
Your consent will be asked when a consultation of your electronic medical file is relevant or when sharing relevant information with your healthcare team.
Please note that you can withdraw your consent at any time or remove your health insurance number from your file at any time, if this is not a mandatory situation.
Data retention
Legally we are required to keep your entire file and billing data for a minimum period of 5 years following the end date of your last meeting with our professional Order (OPPQ) and billing data must be kept for 6 years following the last reference fiscal year. Sending sensitive data by email
Given the sensitivity of data related to your health, we do not recommend that you send us documents such as prescriptions, medical certificates or health insurance numbers by email. Even in 2023, it remains more prudent to do so by sending faxes or to have your documents scanned in person in your patient file by our staff. If you still send us documents containing sensitive information by email, you accept the risks inherent in this method of transmission. Appointment management, invoicing and customer file management software
Your entire physiotherapy file at Clinique Alta Physio is taken care of by Medexa, a Quebec clinic management software company. Thanks to Medexa, your data is safe since it is encrypted and stored on highly secure servers located in Canada. For more information, here is the link to their website: (https://medexa.com)
Website
In order to better serve you, the website cliniquealtaphysio.com collects certain information when you visit it via Google Analytic: number of users, session statistics, approximate geolocation and information on the browser and the device used. This anonymized information will be stored for a period of 26 months on Google servers located in the United States, among other places. In addition, you can refuse markers (cookies) when you visit our website.
Exercise program via Physiotec
To make it easier to monitor your exercises and create your exercise programs, we currently use Physiotec software integrated into our Medexa software. In order to create a Physiotec user profile, your following identification data will be shared with this platform:
Name/Email Address/Phone Number
Name and address of the clinic that provided your care
Your exercise program
The data you enter in the application as a user
Other data included in the documents you share with your professional on your physiotec account

We can also provide you with an exercise program on a homemade paper document if you do not wish to use this functionality or if this method of transmission exercise program is not applicable or appropriate to your personal situation.
